Use this form to request a free, 10-15 minute phone consultation with Dr. Danwar. The purpose of this call is to determine if prospective patients and the physician will be a good fit. Providing this information and participating in the consultation call does not establish a doctor/patient treatment relationship. Dr. Danwar is an out-of-network physician , does not accept insurance, and is unable to accept patients with Medicare. We can provide a superbill for insurance reimbursement. Please note that due to clinical acuity, Dr. Danwar may not be able to accept patients who have active self-harm, have had a suicide attempt in the last 3 months, or have thoughts of harming others.
